GENERAL GUIDELINES FOR MICROBIOLOGICAL Specimen COLLECTION


SPECIMEN FOR
URINE CULTURE




CLEAN VOIDED SPECIMEN

CATHETERIZED STRAIGHT

URETERAL BLADDER TAP
FOLEY OR INDWELLING CATHETER



Use aseptic technique

Clean perineal area or penis with chlorohexidine 4% until free of secretion.

Do not collect first portion of urine, collect 10-20 ml of later flow in sterile container.

To laboratory within 30 minutes or refrigerate immediately and send within 2 hours.

The small volumes for urine culture should be obtain only by needle aspirations of the sample port or the distal end of the catheter. The site should be disinfected with alcohol swab and a sterile syringe is used for aspiration
Larger volumes of urine for other analyses should be obtained aseptically from the drainage bag.








SPUTUM CULTURE

Including AFB (Acid Fast Bacilli)


The best time to collect a sputum sample is early in the morning, before having anything to eat or drink.

The patient should first rinse his or her mouth with water to decrease mouth bacteria and dilute saliva.

Through a deep cough, the patient must cough up sputum from within the chest.

Taking deep breaths and lowering the head helps bring up the sputum.

Sputum must not be held in the mouth but immediately spat into a sterile container.


Note: If coughing up sputum is difficult, a health care worker can have the patient breathe in sterile saline produced by a nebulizer. This nebulizer saline coats the respiratory tract, loosening the sputum, and making easier to cough up.


TRACHEOSTOMY/ETT
CULTURE


Use aseptic technique.
Use sterile suction catheter.
Suction 5-10 seconds rotating the catheter between you thumb and forefinger while slowly withdrawing it.
After the entire catheter has been removed, the distal end of the catheter is aseptically cut and placed in a sterile container of normal saline for immediate microbiologic processing
.
SURGICAL WOUNDS AND WOUNDS CULTURE

Remove superficial debris by gently wiping with cotton soaked in sterile saline.
Swabs collected from infected sites should be transferred slowly and carefully to the swab container so as to avoid contamination of the rim.
To ensure even distribution of microorganism on the collecting material, rotate the swab when taking the specimen.
To lab within 60 minutes

THROAT CULTURE

Sterile swabs provided in the kit.
Collect the throat swab specimen with the sterile swab that is provided in the kit. Swab the posterior pharynx, tonsils and other inflamed areas. Avoid touching the tongue, cheeks and teeth with the swab.
Testing should ideally be performed immediately after the specimens have been collected. Swab specimens may be stored at room temperature for up to four hours prior to testing






NASAL CULTURE

Sterile swabs provided in the kit.

Moisten swab with sterile normal saline or distilled water.

Gently roll the swab for 2-3 seconds over the area.

Send to laboratory immediately.




ABSCESS MATERIAL
Use aseptic technique.

Collect approximately 3ml of pus along with scrapings from the abscess wall is practicable and put in a sterile container.



SKIN LESIONS
Swabs intact pasture with 70% alcohol and allow drying.

Either pierce with a sterile needle and absorb the contents onto a sterile swab or aspirate the contents and express either onto a sterile swab or into a sterile container.





STOOL CULTURE

(Ova and parasites
Fresh stool in a clean container

Do not refrigerate, to lab within 60 minutes.



RECTAL SWABS
Swabs should be inserted just inside the anus and rotated with firm pressure.
On withdrawal it should be placed in a screw cap tube containing a preservative such as Alkaline Peptone Water.



BLOOD CULTURE
Use strict aseptic technique. Wear gloves;they do not have to be sterile.
Apply a tourniquet and locate a fixed vein by touch. Release the tourniquet while the skin is being prepared.
Prepare the skin for venipuncture by cleansing it with 70-95% isopropyl alcohol or 70% ethanol. Using 2% tincture of iodine or an iodophor preparation, start at the venipuncture site and cleanse the skin in concentric circles of increasing diameter. Keep the iodine preparation wet on the skin for at least one minute. Do not touch the skin after has been prepared.
Reapply the tourniquet, perform venipuncture, and withdraw approximately 20ml of blood (for adults).
Add the blood to labeled aerobic and anaerobic blood culture bottles. Take specimens to the laboratory promptly or place them in a incubator at 37ºC.



EAR CULTURE
Best results will be obtained if the ear is actually draining.
Gently rotate swab in affected ear.





EYE CULTURE
Swabs for culture should be taken prior to application of topical antibiotics.

Sample both eyes by rolling separate swabs premoistened with sterile nonbacteriostatic saline over each conjunctiva,
